*There is a one-time, nonrefundable, enrollment fee for memberships of $200. A nonrefundable, re-enrollment fee applies to lapsed memberships.
^There is a nonrefundable booking fee of $100 for Fee-For-Service. This will be credited to your total cost the day of the appointment. For nonmembers (fee-for-service), there is an additional fee of $100 for same-day appointments.
